Medicaid Targeted Case Management (TCM) for Children at Risk of Abuse and Neglect
The Medicaid Targeted Case Management (TCM) for Children at Risk of Abuse and Neglect program is a Florida Medicaid program where federal dollars are matched by locally funded dollars (CBHC contract dollars) for Targeted Case Management services.
Under this TCM program, children and their families who live in Hillsborough County and are Medicaid eligible, receive case management and gain access to an array of services including medical, social, educational and other services that meet their needs.

Thank you for your interest in participating in the Targeted Case Management (TCM) for Children at Risk of Abuse and Neglect Program. If you are a case management program funded by the Children’s Board of Hillsborough County and meet the TCM program certification requirements, you may be certified to participate in the TCM Program. There are a number of steps needing to be completed before participating in the TCM program, including:
- Obtaining approval by the Children’s Board
- Completing the Application
- Completing the Cost Survey
- Completing the TCM Training
- Contacting the Billing Agent if you choose to use their services
- Consulting with Strategy Solutions

- Select Public Information for Providers
- Go to Provider Box on the left
- Select Enrollment
- Go to Enrollment Forms
- Select Florida Medicaid Provider Enrollment Application
Once you have accessed the application, you will need to print it out and fill it out according the Children’s Board instructions. Please contact your Program Consultant or Dr. Chamain Moss-Torres at 204-1733 for further instructions.
TCM Cost Survey
A web application designed to assist agencies in determining the financial impact associated with participating in Medicaid Targeted Case Management for Children At-Risk of Abuse and Neglect. New TCM providers are required to complete the TCM Cost Survey.

TCM Handbook
